Submit a verbal request to the civil status officer.
Pay the applicable fee (in cash or via postal order to the municipal collector, with a stamped envelope including the applicant's address if requesting by mail)

The extract is issued to the applicant, their ascendants, descendants, legal representative, or non-divorced spouse.
Copies of the records may be provided free of charge to the public prosecutor or authorized administrative institutions.
