Just submitting a request is sufficient
In case of marriage:
The applicant must be Tunisian and married
The family record book request can be submitted by: the head of the family, the divorced woman if not remarried, the widow who holds the record unless otherwise decided by the court.
The marriage contract must be concluded in the municipal jurisdiction, either by the municipality or by notaries.

Or a civil status document of the person concerned stating whether it is an original, declaration, or court judgment (for birth extract)
For marriage extract: specify whether the contract was concluded at the municipality or by notaries
Payment of fees for requested documents in cash or by postal order to the municipal treasurer, with a stamped envelope bearing the applicant’s address if the request is sent by mail

Photo of the head of the family
Copy of the national identity card of the husband
Marriage extract
Birth extract of each spouse
Birth extracts of other family members (in case the family record book is issued after some time)

Remarks
Remark 1:
Civil status extracts are issued to the holder of the extract or to their ascendants, descendants, guardian, legal representative, or spouse if not separated or divorced.
Remark 2:
Copies of extracts are provided free of charge to the public prosecutor or other administrative institutions.
